Our Team
We are a group of educators with backgrounds in social work, special education, business and I/DD. Uniquely positioned to provide support coordination services, we are highly qualified staff with expertise in preparing young adults for life after 21. Engaging with our clients, developing an effective plan and communicating and meeting with individuals and families regularly, our role as support coordinators is to help you fulfill your goals.

What do you do?
For individuals who are receiving services from the Division of Developmental Disabilities (DDD), a support coordination agency (SCA) helps manage the services that they will receive.
How do I select a Support Coordination Agency?
You need to fill out an SCA selection form or change form and send it to DDD and the selected SCA will reach out to you.
What makes your agency different?
We have support coordinators who have transition planning experience where they have prepared young adults for the many changes in programs and people they will encounter. In addition, they are invested in each individual and will not only take the time to really get to know them but will most likely be their long-term supporter.
What areas do you cover in New Jersey?
Currently, we service Hudson and Bergen counties.
